Application & Review Process

How to Apply

Requests are only accepted from a patient’s treating oncologist.

To apply, oncologists should:

  • Confirm that the patient meets the criteria for the Case-by-Case Review Program. For criteria, see the Case-by-Case Review Policy for Cancer Drugs .
  • Download and complete the appropriate form (i.e., Request Form or Appeals/Resubmission Form).
  • Review the express consent provision in the Request Form with the patient to ensure the patient is informed of the use and disclosure of their personal health information as outlined.
  • Gather all required supporting documentation.
  • Submit the form and all supporting documentation via our secure upload tool. Get instructions

Review Process

We review each request, in consultation with expert reviewers if necessary, and make a funding recommendation to the Executive Officer (EO) of Ontario Public Drug Programs. The EO reviews each request package we submit and makes the final decision to approve or reject a request for funding.

Estimated Turnaround Time

This program is not intended for acute or emergency therapy. We aim to provide a funding decision within two weeks of receiving a completed request.
